About the Treatment
What is Tear Trough Filler?
Tear trough filler is a non-surgical treatment for hollows and dark shadows beneath the eyes, often called tear troughs. A small amount of hyaluronic acid filler is placed under the eye to restore lost volume and soften the groove between the lower eyelid and the cheek.
With age, the under-eye area and the mid-cheek lose volume, and a ligament that tethers the skin to the bone becomes more visible. Together these create a shadow that can make you look tired even when you're not. Because the cheek and the tear trough work together, both are assessed before treatment so the result looks balanced and natural.
We usually treat this area with a cannula, a fine, blunt-tipped tube that allows precise placement through fewer entry points and can reduce bruising and swelling. Filler isn't always the right answer for under-eye concerns, and prominent eye bags can look worse with it, so we'll tell you honestly at your consultation whether it suits you.

Questions
Frequently Asked
What is the tear trough?
The tear trough is the area between your lower eyelid and your cheek. As we age we lose volume here, which can cause hollows, dark circles and shadows under the eyes.
Why are my tear troughs so deep?
A firm ligament connects the skin to the bone under the eye. As the bone and surrounding tissue lose volume with age, this ligament pulls the skin inwards and the groove becomes more noticeable. It can look deeper still if the fat pads above it bulge forward.
Does tear trough filler hurt?
Most people find it far less uncomfortable than they expected. The small injections can cause mild discomfort, but pain is usually minimal.
How long does tear trough filler take to settle?
You may have some swelling, bruising and redness, which usually improve within 48 to 72 hours. The area looks fuller straight away, but it can take one to two weeks to see the final result.
How should I sleep after tear trough filler?
For the first three nights, sleep on your back with your head raised on two pillows, or use a travel-style neck pillow to keep your head still. This helps the filler settle in place.
Can filler make under-eye bags worse?
It can. Filler is not usually the right treatment for prominent eye bags and may make them look larger or bluish. We assess this carefully and will suggest alternatives if filler isn't suitable for you.
Can tear trough filler look lumpy?
Some unevenness is common in the first few weeks and usually settles within two to three weeks. If anything remains after that, Dr Alexa may recommend gentle massage and will show you how to do it.
How long does tear trough filler last?
Typically 10 to 12 months, and sometimes longer, because there is little muscle movement in this area.
What are the side effects?
Mild swelling, redness and bruising are common for a few days, and occasionally bruising lasts up to ten days. Serious complications are rare, which is why this delicate area should only be treated by a medically trained practitioner.
Can I have tear trough filler if I'm pregnant or breastfeeding?
No. Dermal fillers haven't been tested for safety in pregnancy or breastfeeding, so we wait until afterwards.
Can I have tear trough filler if I'm under 18?
No. In the UK it is illegal to give dermal fillers for cosmetic purposes to anyone under 18, even with a parent's consent.
